Appointment Scheduling Instructions

1. Create Your Follow My Health Account
   Visit www.followmyhealth.com to create your secure, HIPAA-compliant patient portal account.
2. Connect to the Correct Facility
   After your account is created, search for and connect with:
   AFB Nursing and Services
3. Wait for Approval
   You will receive a confirmation email once AFB Nursing and Services approves your connection request.
4. Schedule Your Appointment
   In the top portion of your portal screen, select Schedule an Appointment.
5. Select Appointment Type
   Choose either:
   • Telehealth – Initial Visit
   • Telehealth – Follow-Up Visit
6. Select Your Provider
   Click on Ms. Arneta Finney.
7. View Calendar
   Select View Calendar.
8. Choose a Day
   Appointments are available on Wednesdays only.
9. Choose a Time
   Select an available time slot and click Continue.
10. Reason for Visit
    Briefly state why you are scheduling an appointment with the nurse practitioner.
11. Submit
    Submit your request to finalize your appointment.

Important Reminders

 Virtual Appointments Only

 All appointments are conducted exclusively via telehealth.
No in-person visits are currently available.

Payment & Intake Requirements 

To avoid a no-call / no-show fee, the following must be completed 24 hours before your visit:

• Zelle payment of $250.00
  Send to: aarneta1@sbcglobal.net
• Intake form completed and signed
• Valid driver’s license
  Upload through Follow My Health or email to:
  contact@afb-services.net

Telehealth Link

A secure telehealth link will be sent to you via text or email around your appointment time.

Please note:
The link may arrive up to 30 minutes before or up to 30 minutes after your scheduled time. No need to call asking for the link untill 30 minutes pass your appointment time.
