AFB Nursing & Services, P.C. – Office Policies

Payment & Insurance 

  • If you do not use insurance, payment is due 24 hours before the visit unless other arrangements are made with AFB NURSING and SERVICES PC.
    For insured patients, you are responsible for applicable copays, deductibles, and noncovered services.
  • We submit claims to your insurer one time. If a claim is denied for any reason, the remaining balance is your responsibility. 
  • Please notify us of any changes to insurance, address, phone, or email before your visit. 

Fees 

  • Forms fee: Nonrefundable, starting at $65 per set (e.g., Disability, FMLA, AFLAC, Medical Record Request, etc.). Processing time: 7–10 business days after payment (no refunds). 
  • Virtual evaluation visit: $250 nonrefundable. Payment is due ≥24 hours before the visit or the appointment may be cancelled (no refunds). 
  • No-show/late cancel: $250 if you miss or cancel within 24 hours of the scheduled time (no credit given). Reschedules within 24 hours may incur an additional fee. 
  • Payments accepted: Zelle to AARNETA1@sbcglobal.net.

Telehealth Only (No in-Person Visits at This Time) 

  • Visits are conducted via a HIPAA compliant video platform. By participating, you acknowledge that health information may be discussed electronically. 
  • Telehealth may not be appropriate for all concerns. If needed, we may recommend an in-person evaluation.
  • Services are provided only to patients physically located in California and Nevada at the time of service. 
  • You may withdraw consent for telehealth at any time; this will not affect your right to future care. 

Scheduling, Cancellations & Communication

  • A no-show is missing your appointment without notice. A late cancellation is cancelling < 24 hours before your appointment time. 
  • Routine portal messages are reviewed during business hours and answered within 1–2 business days. For emergencies, call 911; for mental health crises, call 988. 
  • Patients who use FollowMyHealth may receive reminders by text/email. Reply STOP to opt out. Message/data rates may apply. 

Prescriptions & Refills 

  • Request refills at least 3 business days before you run out. 
  • We may require an appointment for medication changes or if you have not been seen within the last 6 months
  • Controlled substances are prescribed only when clinically appropriate and in accordance with applicable laws and telehealth regulations. 

Medical Records & Forms 

  • Submit medical record requests in writing. Standard processing time is 7–10 business days. Copy fees may apply where permitted by law. 
  • Form completion begins after the forms fee is paid. Provide all required sections and signatures to avoid delays. 

Insurance Release & Privacy 

  • I authorize insurance benefits to be paid directly to AFB Nursing & Services, P.C., and I am financially responsible for any balance for any reason. 
  • I authorize AFB Nursing & Services, P.C., and its business associates to release the minimum necessary information to process claims. 
  • I have had the opportunity to review the Notice of Privacy Practices. I may request restrictions or revoke consent for disclosure in writing. 

Good Faith Estimate (No Surprises Act) 

  • Uninsured or self-pay patients are entitled to a Good Faith Estimate of expected charges and may request one at any time. 

Telehealth Recording (Optional) 

  • With your written consent, portions of a telehealth session may be recorded solely for treatment and documentation. Recordings are stored securely, not used for marketing, and may be revoked in writing at any time. If you do not consent, visits will not be recorded. 

Accessibility & Conduct 

  • Interpreter services and reasonable accommodations are available upon request at no cost. We maintain a zero tolerance policy for abusive, discriminatory, or threatening behavior. 

Agreement to Terms/Consent to Treat 

  • By scheduling or receiving services, you acknowledge and agree to these policies and consent to treatment by AFB Nursing & Services, P.C., including our nurse practitioners, physicians, mental health clinicians, and medical staff.
